We are delighted to be rolling out the Covid 19 vaccine in the coming weeks from our practice in Collins Avenue commencing the week of the 1st of March 2021.
We will be providing the vaccine to all of our most vulnerable patients first starting with the over 85 age group and working progressively through younger ages and more vulnerable groups. We are strictly bound to providing the vaccine to the most vulnerable groups first as per HSE advised and no exceptions will be made to this order.

While it is up to you to decide to get the vaccine, the HSE strongly recommends that you get the vaccine as soon as we offer it to you. The HSE is offering the vaccine free of charge.
There is no need to contact the practice about the vaccine as we have generated a list of the priority groups and will contact you when it is your turn. We are dependent on the supply of the vaccine from the HSE and we have significant numbers of patients over the age of 70, so the process will take time.
What’s important is that you inform yourself or your elderly relative about the vaccine in advance of getting it. The vaccine that we are currently being provided with is the Pfizer/BioNTech vaccine and all the information on this vaccine including information leaflets is available on the HSE website www.hse.ie
We would also appreciate that you print off and sign the consent form provided below and bring it with you on the day of your vaccine to help speed up the process. If you don’t have access to a printer hard copies of the form will available at both Baldoyle and Collins Avenue practice.
