As a first time patient, we ask that you make an appointment.
On arrival to the Surgery, you will be asked to complete:
- a short form registering your details with the administration staff
- a Release of Records form so we can retrieve your medical records from your previous doctor
If you hold an existing GMS Medical card and you wish to transfer to Dr. McEvoy’s Surgery, we ask that you make an appointment to visit with Dr McEvoy (this is not a consultation).
After your visit with Dr McEvoy, if there is available room on our GMS list, you will be asked to fill out our registration form and a release of records form.
